1. Minimum of 30 principal members.
2. Maximum entry age of 65 [any members aged 60-65 can only attribute to up to 5% of the total group]
3. Prepared to accept hospital list.
4. Previous Claims and premium information will be required prior to acceptance by Revelation.

If you can accept these four points then you will be eligible to set up your very own Health Trust Fund Scheme.

To form your own Trust Fund Scheme [currently NOT Insured]

1. Minimum of 30 principal members.
2. Maximum entry age of 65 [any member’s aged 60-65 can only attribute to up to 5% of the total group]
3. Prepared to accept hospital list.
4. All members will complete a health declaration and have an 18 month moratorium applied to existing conditions.

Providing this period remains treatment free then the condition will be eligible for treatment after the 18 month has expired. If you can accept these points then you will be eligible to set up your very own Health Trust Fund Scheme.
